If you answered YES to any question, you are considered INDEPENDENT and must submit your most recent SIGNED Federal Income Tax form 1040.

Participant Commitment:

The TRIO SSS Program only works if students participate. If you are selected for the program we require the following commitment from you:

  • I WILL participate in tutoring and/or mentoring sessions
  • I WILL participate in at least one (1) Personal Education Plan session with my Academic Counselor per semester
  • I WILL attend at least two (2) TRIO SSS workshops per semester
  • I WILL attend at least one (1) TRIO SSS event per semester
  • I WILL maintain a cumulative GPA of 2.0 or higher 

I will participate in the TRIO SSS services listed above, which are designed to help me reach my academic goals, career goals, and promote cultural growth. 

By signing below, I give permission to Tulsa Community College to release all records relevant to my financial and academic information to the TRIO SSS Program. 

I verify that the information provided in this application is correct and complete to the best of my knowledge.

I give the TRIO Student Support Services program permission to use my statements, images, voice, or combination of these in promotional items and contact me by text, phone, email, fax, or mail.
